We offer a Calendly-like solution for hospitals scheduling Medicare patients’ post-acute care. 


Problem

Our team targets healthcare communication silos to enhance interoperability between hospitals and outpatient primary care physicians (PCPs), particularly for patients without assigned PCPs. Independent hospitals and practices, comprising 25% of the healthcare community, struggle with coordinating patient care due to varied electronic health systems (EHRs), resorting mainly to fax and phone calls.

Our solution streamlines post-acute visit scheduling for outgoing hospital patients, reducing coordination efforts from 4 phone calls to 4 clicks for hospital care coordinators. We integrate with diverse EHRs to ensure a user-friendly experience. The lack of technological interoperability in healthcare leads to significant administrative costs (estimated near $250B) and poorer health outcomes, especially for seniors.

A PCP visit post-hospital discharge significantly lowers hospital readmission rates, which are costly (averaging $6 million annually per hospital).
